To make the caramel layer, pour the caramels and 1/4 of heavy cream into a microwave-safe bowl. Heat for 1 minute, then stir well. Heat for 20-30 second increments, stirring in between each heating period, until the caramel is smooth.
To make the chocolate ganache layer, pour 1/4 of heavy cream into a microwave safe bowl and heat it for 45 seconds or until the cream begins to bubble around the sides of the bowl (you don't want it to get so hot that it begins a rolling boil).
Remove the bowl from the microwave and pour the chocolate chips over the hot cream. Allow it to sit for about 30 seconds, then stir well. Return the bowl to the microwave in 30 seconds increments, if necessary, until the chocolate is completely smooth.
